Referrals and Fees

 

PATCHES provides diagnostic and therapy services for complex developmental delay and disability issues.
Eligibility for referral to diagnostic services include:
  • Australian citizen or permanent resident
  • Engaged in the Child Protection or Justice system (agencies are required to fund diagnostic assessments)
    • Rural Health West-funded clinics – may include children not engaged in the child protection or justice system
  • Ability to fund assessment (e.g. private or agency-funded, referred through a host agency including Aboriginal Medical Service or school, or partner organisation)
  • Aged 0-18 years (adult assessments for individuals in the Justice system up to 35 years are offered)
  • Referred to PATCHES by a GP and / or paediatrician using a GPMP, TCA and/or GPMHCP

DIAGNOSTIC REFERRAL PROCESS:

STEP 1:

  • Click on this link PATCHES Referral Form, export as PDF, fill and sign electronically and email
  • The parent/carer/guardian must sign the form

STEP 2:

  • GP referral:  At your GP appointment ask the GP to click on this link PATCHES GP Referral Form Form, export as PDF.  Determine eligibility for a GP Management Plan and/or Team Care Arrangement, and electronically email
  • A referral from a GP is compulsory and must be emailed to PATCHES prior to confirmation of an appointment

If unable to refer electronically, please:

  • Fill, scan and email both completed referral forms to PATCHES
  • PATCHES will consider the referral and make contact to confirm an appointment

 

Eligibility for referral to therapy services include:
  • Australian citizen or permanent resident
  • Engaged in the Child Protection or Justice system
  • Eligible for DSC or NDIS therapy funding
  • Ability to fund therapy (e.g. private or agency-funded, referred through a host agency including Aboriginal Medical Service or school, or partner organisation)
  • Client aged 0-18 years, or family/carers of a child or adult
  • Referred to PATCHES by a GP and / or paediatrician using a GPMP, TCA and/or GPMHCP
  • Diagnosed with a complex developmental or disability condition:
    • Specific therapy programs for:
      • Fetal Alcohol Spectrum Disorder (12 sessions) and/or,
      • Early Life Trauma/attachment disorder (12 sessions), or
      • Intensive Trauma – EMDR (2-3 sessions)
    • Intervention for Disability services eligible conditions including:
      • Autism
      • Global Developmental Delay
      • Intellectual Disability, Cerebral Palsy, Down Syndrome
      • Other NDIS, disability services and school disability services eligible conditions
      • Other
